RN Director, LTSS Clinical Care Management

For our client, we are seeking a RN Director, LTSS Clinical Care Management to join the team of a leader in the Hospitals & Medical Centers space.
This role will lead long-term services and supports clinical care management with a strong focus on quality, cost effectiveness, and member outcomes. The Director will guide team goals, oversee care management operations, and ensure alignment with state requirements and quality standards. This position will also partner with vendors and internal stakeholders to strengthen clinical pathways, support staff performance, and improve overall care delivery.
Location: Remote - US based candidates only, no visa sponsorship available
Compensation: $133,700 – $247,400 annually
Responsibilities
Lead the long-term care management process to enhance quality and cost-effectiveness
Set and monitor goals for the care management team based on quality metrics
Ensure compliance with corporate and NCQA standards in long-term care management policies
Oversee staff assignments and caseload management in alignment with state regulations
Conduct vendor oversight and manage contractual reporting as needed
Develop strategies to improve clinical care pathways and reduce healthcare costs
Provide coaching and support to enhance team performance and member satisfaction
Qualifications
Registered Nurse (RN) license required; Compact Nursing License acceptable
Graduate of an accredited School of Nursing with 7+ years in clinical nursing experience
Minimum 4 years of leadership experience in managed care or a related field
Expertise in Value-Based Care Management, particularly in Child Welfare or Medicaid programs
Strong clinical knowledge, including patient assessment and care planning
