Health Care Navigator

Doctorite is seeking a compassionate and organized Health Care Navigator (HCN) to support Medicare Advanced Primary Care Management (APCM) and Behavioral Health Care Coordination programs. In this remote role, you will engage patients with chronic medical and behavioral health conditions, conduct monthly care coordination activities, develop care plans, facilitate referrals, and serve as a vital link between patients, providers, and community resources. The HCN will work closely with primary care practices and clinical teams to improve patient engagement, care quality, and health outcomes.
Key Responsibilities
Conduct monthly patient outreach, assessments, and care coordination activities.
Doctorite AI platform generates notes and patient care plan goals for your review and approval
Coordinate referrals, appointments, and access to behavioral health, specialty, and social services using in built telehealth functions
Maintain communication with patients, families, providers, and care teams via our texting and email functionalities
Utilize Doctorite technology platforms to deliver virtual care coordination services.
Qualifications
Bachelor’s degree in Healthcare, Social Work, Psychology, Nursing, Public Health, or a related field.
Minimum 2 years of experience in care coordination, case management, patient navigation, behavioral health, or a related healthcare setting.
Strong communication, active listening, and relationship-building skills.
Knowledge of Medicare, CMS programs, chronic disease management, and behavioral health concepts.
Experience working with EHR systems and healthcare technology platforms.
Understanding of mental health conditions, including depression, anxiety, PTSD, and substance use disorders.
Proficiency with Microsoft Office and virtual care delivery tools.
Ability to maintain patient confidentiality and work effectively with diverse populations.
Must be Bilingual and fluent in English and Spanish
