Role Description This is a contract, remote role for Experienced Home Health Nurses. Day-to-day responsibilities include performing comprehensive patient assessments, developing and updating individualized care plans, and delivering skilled nursing services in patients’ homes via in-person visits and/or virtual care as applicable. Nurses will monitor symptoms, administer medications, provide wound care and other clinical interventions, and coordinate with physicians, therapists, and caregivers to ensure continuity of care. The role includes documenting all care in accordance with regulatory and agency standards, educating patients and families on disease management and safety, and escalating changes in condition promptly. Nurses are expected to manage their schedules effectively, maintain professional communication, and use electronic health record systems to support accurate, timely documentation.
Qualifications
Active, unrestricted Registered Nurse (RN) license in the relevant state(s) of practice; eligibility to work as an independent contractor.
Demonstrated experience in home health or community-based nursing, including patient assessment, care planning, and case management.
Strong clinical skills in medication management, wound care, chronic disease management, and post-acute care support.
Proficiency with electronic health records and remote documentation; comfort using telehealth and digital communication tools.
Excellent communication and interpersonal skills, with the ability to educate patients and caregivers in clear, accessible language.
Ability to work independently, prioritize multiple cases, and make sound clinical judgments in varied home environments.
Knowledge of home health regulations, documentation standards, and quality and safety practices; experience with Medicare/Medicaid guidelines is beneficial.
Reliable internet access, secure workspace for remote coordination, and, where applicable, reliable transportation for home visits.
Current CPR/BLS certification and applicable immunizations or health clearances as required by local regulations and client partners.
Experience serving diverse populations and a commitment to culturally responsive, non-discriminatory care.
