ABOUT ALLERMI
Allermi is a direct-to-patient consumer health company built around a prescription allergy nasal spray. We manage the full patient journey in-house: enrollment, medical intake, provider review, prescribing, subscription commerce, and pharmacy fulfillment. Our technology grew up quickly around real patient and pharmacy volume rather than being built ahead of it. That's the opportunity: the fundamentals work, but the systems, data, engineering org, and consumer experience need a first real technical leader to take them from "working" to best-in-class, with AI and agentic systems as a core part of how we build and operate.
THE ROLE
We're hiring Allermi's first full-time Head of Engineering. This is a hybrid builder/leader role: in your first months, you'll audit our engineering team, codebase, third-party systems, and workflows end to end, then turn that assessment into a technical roadmap and start building. You'll own three interconnected platforms (the patient-facing subscription commerce experience, internal operations tooling, and pharmacy/fulfillment integrations) and build the team and AI-native practices to support them as we scale.
You'll report to the CEO and work closely with operations, data, clinical, growth, and support leadership. Expect to spend real time in the codebase, actively use modern AI development tools, and ship, not just operate from planning docs.
WHAT YOU'LL OWN
Evaluate & Assess
Audit the engineering team's structure, skills, workflows, and capacity; decide what to keep, restructure, or hire around
Review the codebase, infrastructure, and architecture across the patient app, internal tools, and pharmacy integrations for scalability, reliability, and security gaps
Map every system of record and third-party dependency (commerce, subscription billing, intake, prescribing workflow, support, pharmacy integrations, fulfillment, and our Databricks warehouse) and document how data and patient states actually flow
Turn technical debt, single points of failure, and AI/automation opportunities into a prioritized roadmap
Patient Platform
Own the end-to-end digital patient experience: enrollment, medical intake, prescription and refill flow, checkout, recurring billing, and self-service subscription management
Reduce friction and drop-off across the funnel; partner with growth on conversion and retention; modernize the patient portal
Streamline the intake-to-prescription handoff with clinical stakeholders, including the shift toward an NP/PA prescribing model
Agentic & AI-First Engineering
Make AI-assisted development the default across coding, testing, review, and documentation, with clear standards for quality, security, and human oversight
Build agentic workflows that remove repetitive operational work in support, clinical ops, pharmacy coordination, reporting, and reconciliation, prioritizing measurable business impact
Design the APIs, data access, observability, and evaluation frameworks that let AI agents safely interact with regulated data while preserving auditable human control
Continuously evaluate emerging AI models and tooling, and move fast when something materially improves velocity or leverage
Internal Platform
Replace spreadsheet- and ops-run processes with engineering-owned internal tooling for operations, support, and the provider review queue
Move recurring reporting (flash reports, open orders, refill forecasting, cancellation analysis) onto durable infrastructure built on Databricks
Implement data access, auditing, and security practices appropriate for PHI/HIPAA
Pharmacy & Fulfillment Platform
Own the integration layer with our compounding pharmacy partners (order transmission, status sync, exception handling) and extend it as we add partners
Lead the technical evaluation of our current pharmacy-side software: replace, extend, or build in-house
Improve order-status visibility so operations and support can trust order status without manual cross-checking
Team & Organization
Recruit, hire, and mentor engineers as the team grows, including roles already in flight
Establish engineering practices (AI-assisted development standards, code review, testing, deployment, incident response, security) fit for a regulated, patient-facing product
Set a clear technical roadmap and give leadership visibility into priorities and trade-offs
WHAT WE'RE LOOKING FOR
7+ years in software engineering with several years in technical leadership, ideally at an early-stage startup
A track record of walking into an existing codebase and team, assessing it honestly, and executing a roadmap, not just greenfield building
Strong full-stack depth and a willingness to stay hands-on
An AI-first mindset: you actively use coding agents and LLM tooling to increase your own output, and you redesign team workflows around those capabilities
Experience deploying AI-powered or agentic systems in production, or the depth and urgency to become excellent at it fast
Experience building both customer-facing products and internal tooling, ideally in consumer subscription or e-commerce with physical fulfillment
Comfort integrating third-party systems and vendors you don't control
Working knowledge of HIPAA/PHI and secure systems in a regulated environment
A collaborative style: this role sits at the center of clinical, ops, data, and support
NICE TO HAVE
High-growth DTC, telehealth, healthcare, or pharmacy experience with recurring revenue and physical fulfillment
Hands-on Shopify (or similar), subscription/payments infrastructure, Databricks, and modern AI developer tooling
Experience using AI to automate back-office workflows or build internal agents
Pharmacy management, compounding, or e-prescribing exposure
Scaling an engineering team from a handful of engineers into a department
WHAT SUCCESS LOOKS LIKE
90 days: Full assessment of team, codebase, systems, and AI opportunities complete; prioritized roadmap shared with leadership; trusted relationships built across functions.
6 months: Clear plan and early progress on the pharmacy software decision; measurable improvements shipped in intake-to-prescription and subscription journeys; at least one major operational workflow moved onto engineering-owned infrastructure with an AI/agentic approach; reliable instrumentation across the subscription funnel.
12 months: Patient, commerce, internal, and pharmacy platforms are stable, documented, and built to scale; a right-sized, high-output team is in place with an AI-first culture and a roadmap the whole company trusts.
