Home Health RN/LPN – Part-Time / Per Diem / PRN
A&Z Healthcare Services Inc. is seeking compassionate, dependable, and professional Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our growing home health team on a Part-Time, Per Diem, and PRN basis.
This is an excellent opportunity for nurses who enjoy providing personalized, one-on-one care while helping patients remain safe and independent in the comfort of their own homes.
Job Summary
The Home Health RN/LPN provides skilled nursing care to patients in their homes in accordance with the physician’s orders, established plan of care, and agency policies and procedures.
Our nurses work independently while communicating with physicians, supervisors, caregivers, and other members of the healthcare team to ensure patients receive safe, compassionate, and high-quality care.
Responsibilities
Provide skilled nursing care to patients in their homes
Perform patient assessments, including vital signs and evaluation of the patient’s condition
Monitor patients for changes in condition and report significant findings appropriately
Administer medications and treatments as ordered
Perform wound care and other skilled nursing procedures as ordered and within scope of practice
Provide catheter, ostomy, feeding tube, and other nursing care as applicable to the patient’s plan of care
Educate patients and caregivers regarding medications, treatments, disease management, safety, and the plan of care
Communicate and coordinate with physicians, supervisors, caregivers, and other members of the healthcare team
Complete accurate and timely clinical documentation
Follow infection-control and patient-safety practices
Maintain patient confidentiality and comply with HIPAA requirements
Provide compassionate, respectful, and patient-centered care
Follow all A&Z Healthcare Services Inc. policies and applicable state and federal regulations
Qualifications
Current and unrestricted RN or LPN license
Current CPR/BLS certification
Valid driver’s license and reliable transportation
Strong clinical assessment and communication skills
Ability to work independently in a home care environment
Dependable, organized, and committed to providing quality patient care
Home health experience preferred
