NHTD/TBI Service Coordinator bilingual English/Spanish

Role Description The NHTD/TBI Service Coordinator is a full-time remote role responsible for coordinating services for waiver participants to support safe and independent community living. On a day-to-day basis, the Service Coordinator conducts assessments, develops individualized service plans, and regularly monitors participants’ needs and progress. The role involves scheduling and facilitating meetings, collaborating with providers and caregivers, and ensuring services are delivered according to program guidelines. The Service Coordinator documents all contacts and updates in a timely, accurate manner, responds to participant inquiries, and advocates for needed resources and supports. This position requires dependable remote work practices, consistent communication via phone and video, and adherence to regulatory and agency policies.
Qualifications
Strong Service Coordination skills, including assessment, care planning, and monitoring of NHTD/TBI waiver services.
Excellent Interpersonal Skills and Communication abilities to build trust with participants, families, and providers.
Experience with Operations Management and Customer Service to manage caseloads, meet deadlines, and respond effectively to participant needs.
Knowledge of New York NHTD/TBI waiver programs, community-based services, or related human services programs is preferred.
Ability to work independently in a remote setting, with strong organization, time management, and documentation skills.
Proficiency with basic computer applications, secure email, and electronic record-keeping systems.
Relevant education in social work, human services, psychology, or a related field; equivalent experience may be considered.
Experience working with individuals with disabilities, brain injury, or older adults is a plus.
