Position Overview
Accessible Home Health Care is seeking experienced, dependable, and highly organized Registered Nurses (RNs) to join our field clinical team throughout South Florida.
This role is ideal for an RN who is comfortable working independently in the home setting and has strong skills in comprehensive nursing assessments, medication reconciliation, clinical documentation, care planning, and multidisciplinary coordination.
We are specifically seeking nurses with experience completing Non-OASIS Skilled Assessments and/or Private Duty Initial Admission Assessments. This is not an OASIS-based role. The focus is on conducting thorough assessments, identifying clinical and safety needs, developing appropriate Plans of Care, completing accurate electronic documentation, and coordinating follow-up with providers, pharmacies, caregivers, and the agency Clinical Team.
Key ResponsibilitiesSkilled Assessments & Documentation
Independently complete comprehensive Non-OASIS Skilled Assessments in patients' homes.
Perform thorough head-to-toe nursing assessments.
Assess and document medical history, diagnoses, allergies, functional status, cognitive/mental status, safety risks, and current clinical needs.
Complete accurate medication reconciliation and identify medication discrepancies, concerns, or required follow-up.
Verify PCP/provider, pharmacy, medication, diagnosis, allergy, and order information.
Identify missing, incomplete, or unclear provider orders and initiate appropriate follow-up.
Develop and update individualized Plans of Care based on assessment findings, patient needs, and provider orders.
Complete supervisory visits, reassessments/recertifications, skilled nursing visits, medication-management visits, and discharge assessments as assigned.
Write clear, detailed skilled nursing narratives supporting assessments, interventions, patient response, education, changes in condition, and follow-up needs.
Complete documentation accurately and within agency timeframes using the agency's EMR/EHR.
Private Duty Admissions
Complete Private Duty Non-Skilled Initial Admission Assessments for patients receiving private-duty home care services.
Assessments may include:
Baseline health and functional status
Activities of Daily Living (ADLs)
Mobility and fall risk
Home and environmental safety
Cognitive status
Caregiver support and needs
Medication profile/review, as applicable
Required level and schedule of care
Development of an individualized non-skilled care plan
Care Coordination
Our field RNs are an important clinical link between the patient, family, providers, and agency team.
Responsibilities include:
Communicating professionally with physicians/PCPs, specialists, pharmacies, case managers, caregivers, therapists, and agency clinical staff.
Contacting providers to obtain, clarify, and follow up on orders.
Working with pharmacies to clarify medication lists, refill status, discrepancies, and medication-related concerns.
Following through on outstanding orders, medication issues, abnormal findings, changes in condition, and other clinical concerns.
Promptly escalating significant findings and patient-safety concerns to the appropriate provider and Clinical Team.
Maintaining clear communication with patients and caregivers regarding the Plan of Care and clinical recommendations.
Medication Management
Candidates should be comfortable with medication reconciliation and medication-management responsibilities, including:
Maintaining accurate medication profiles.
Assessing medication adherence, storage, administration, refill needs, and patient/caregiver understanding.
Identifying discrepancies, duplicate therapies, missing or expired medications, and potential medication-safety concerns.
Coordinating medication concerns with patients, caregivers, pharmacies, providers, and the Clinical Team.
Providing medication education and documenting patient/caregiver understanding.
Completing medication-management visits and pillbox/medication-organizer support when ordered and within RN scope and competency.
Clinical Skills
Depending on patient needs and individual RN competency, field nursing responsibilities may include:
Comprehensive nursing assessments and vital-sign monitoring
Medication administration and management
Patient and caregiver education
Wound assessment, measurements, documentation, dressing changes, and wound care
Pressure injury assessment, prevention, and skin-integrity management
Diabetic assessment and management
Blood glucose monitoring and insulin administration
IM and subcutaneous injections
IV assessment/management and PICC/central-line care
Foley catheter assessment, care, insertion, and changes
Ostomy/colostomy care
G-tube/PEG care and enteral feeding
Tracheostomy assessmentOxygen and respiratory care
Pain and neurological assessments
Cognitive, fall-risk, and home-safety assessments
Infection prevention and recognition of signs and symptoms of infection
Recognition, documentation, and reporting of changes in condition
Candidates are not required to have experience with every clinical skill listed above. We ask applicants to clearly identify their current competencies and any areas in which additional training may be needed.
Qualifications
Required:
Current and unrestricted Florida Registered Nurse (RN) license
Strong comprehensive nursing assessment skills
Strong clinical documentation and nursing narrative skills
Experience using electronic medical records (EMR/EHR)
Medication reconciliation and medication-management experience
Strong organization, communication, clinical judgment, and follow-through
Ability to work independently in patients' homes
Ability to efficiently manage assigned field visits and documentation
Valid driver's license and reliable transportation
Willingness to travel within the selected service area(s)
Strongly Preferred:
Previous home health or private-duty nursing experience
Experience independently completing Non-OASIS Skilled Assessments
Experience completing RN admissions, reassessments, recertifications, supervisory visits, and/or discharge assessments
Experience coordinating directly with physicians, pharmacies, case managers, and other members of the multidisciplinary care team
Bilingual English/Spanish
Who We're Looking For
The ideal RN understands that a successful home-care assessment goes beyond checking boxes.
We are looking for nurses who can walk into a patient's home, independently evaluate the clinical situation, recognize risks and unmet needs, reconcile medications, determine appropriate follow-up, develop an individualized Plan of Care, communicate effectively with providers and caregivers, and document the entire encounter thoroughly and accurately.
You should be comfortable taking ownership of the assessment process and following clinical issues through to appropriate resolution.
Above all, we're looking for nurses who combine strong clinical judgment with compassion, professionalism, reliability, and attention to detail.
Assessment Types
Non-OASIS Skilled Assessment
A comprehensive RN assessment for patients receiving skilled nursing services in the private-duty/home-care setting. Responsibilities include clinical assessment, medication reconciliation and management, provider and pharmacy follow-up, review and coordination of orders, Plan of Care development, skilled documentation, and multidisciplinary coordination.
OASIS completion is not required for this role.
Private Duty Non-Skilled Initial Admission Assessment
An initial RN assessment for patients receiving non-skilled private-duty home-care services. The assessment focuses on baseline health and functional status, ADLs, safety, caregiver needs, service needs, medication profile/review as applicable, and development of an individualized non-skilled care plan.
