Aged Care Community Nurse – Part Time | Northern Rivers

Right at Home Northern Rivers is seeking an experienced and compassionate Registered Nurse to join our growing community care team. This is an opportunity to use your clinical expertise in a role where you can build genuine relationships with clients, work autonomously, and make a direct difference to their health, independence and quality of life.
Working 3–4 days per week, this part-time position offers the opportunity to progress to full-time as our services continue to grow.
About the Role
You will provide person-centred clinical care to clients in their own homes, supporting their health, independence and quality of life.
Key responsibilities include:
Clinical assessments, reviews and care planning.
Wound care and medication management.
Monitoring and responding to changes in client health.
Supporting clients with complex and chronic conditions.
Liaising with GPs, allied health and other health professionals.
Providing clinical guidance to Care Partners.
Accurate clinical documentation and incident reporting.
Promoting safe, respectful and person-centred care.
About You
You are an experienced RN who is confident working independently in the community and passionate about aged care.
You will have:
Current AHPRA registration.
Aged care, community or similar clinical experience.
Strong assessment, communication and clinical documentation skills.
Experience with wound care, dementia and chronic conditions.
Current driver's licence and reliable vehicle.
Current First Aid & CPR.
Relevant Police Check and NDIS requirements, where applicable.
