About the Opportunity:
The COMMUNITY HEALTH NAVIGATOR plays a critical role in engaging and enrolling members into the care program, managing a caseload, and supporting members toward their health goals. This position is responsible for building trusting relationships, overcoming barriers to care, and ensuring members complete necessary health activities. The role contributes significantly by facilitating access to health resources and coordinating care, directly impacting the program's effectiveness and member well-being.
Responsibilities:
Conduct phone and in-person outreach to enroll members in the care model
Manage a caseload of 80-120 members, providing coverage during absences or workload spikes
Assess member needs, including social determinants of health, to develop member-centered care plans
Establish trusting relationships and provide timely follow-up
Support members through health activities with empathy and consistent progress monitoring
Coordinate referrals and assist with appointment scheduling and attendance
Organize and prioritize workload to meet monthly goals and daily metrics
Engage members via multiple outreach channels including phone, text, email, and in-person visits
Connect members to community and health resources, including transportation support if needed
Maintain professional boundaries and confidentiality, consulting the multidisciplinary team as necessary
Document all member interactions and keep care plans current in the designated app
Comply with HIPAA and protected health information standards
Work independently and collaboratively in a dynamic environment
Represent the organization respectfully and build community resource relationships
Requirements:
Solutions-oriented with ability to find or improvise pathways forward
Strong verbal and written communication skills
Comfortable learning and troubleshooting technology on mobile and desktop
Self-motivated to manage a remote caseload with minimal oversight
Committed to fostering team connection including participation in occasional in-person meetings
Minimum 1 year direct experience with the served population or equivalent
Bachelor's degree in Social Work, Human Services, Psychology, Health Sciences, or equivalent experience
Experience in crisis planning, case management, and maintaining professional boundaries
Knowledge of community resources and health system navigation
Ability to work as part of a multidisciplinary team
At least 18 years old
Available Monday through Friday during core hours
Valid driver's license, insurance, and reliable transportation if field-based
Pass pre-employment drug screening and background check
Compliance with patient confidentiality and HIPAA regulations
Ability to use technology daily
Ability to lift and carry up to 25 pounds occasionally
