Bilingual Long-Term Care Service Care Manager

Pay Range: $25 - $29/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
Manage a caseload of 50-70 members, focusing on geriatric long-term care.
Conduct assessments with members, caregivers, or providers to develop care plans.
Monitor service delivery and follow up with members through in-person visits and telephonic contact.
Authorize and coordinate referrals for services, ensuring no gaps in care.
Collaborate with discharge planners, physicians, and other stakeholders for effective care coordination.
Assist members with filing and resolving complaints and appeals.
Maintain compliance with new contract requirements effective 2/1/25.
Key Requirements and Technology Experience:
Key Skills; Experience with discharge planning.
Familiarity with TruCare software.
2+ years of care management experience, with a strong emphasis on field experience.
Bachelor's degree in a relevant field (e.g., Psychology, Sociology, etc.).
Valid Driver’s License.
Experience managing high caseloads in a fast-paced environment.
Proficiency with electronic medical health records.
Bilingual in Spanish is required.
