About the role
Description supplied by the original job listing.
Company Description Up & At Em Community Initiative partners with community organizations to improve how individuals and families access and use local resources. The organization helps clients identify needs, navigate service systems, connect with appropriate providers, prepare for appointments or applications, and follow up on referrals. Support spans areas such as housing and shelter, recovery and treatment, mental and physical healthcare, food assistance, employment, and broader community resources.
Partnership arrangements are tailored to each organization’s population and capacity, including case-management support, referral coordination, and scheduled client check-ins.
Services are non-clinical and focus on compassionate, practical support and clear coordination, complementing—but not replacing—emergency response, clinical treatment, legal representation, or services provided by partner organizations.
