This is a part-time, day shift position with flexible hours.
Job Summary
The Home Health Aide provides compassionate, dependable personal care and support to clients in their homes under the direct supervision of an RN. This position plays an important role in helping clients maintain their comfort, dignity, independence, and quality of life.
Qualifications
Must be at least 18 years of age.
Must be a Certified Nursing Assistant (CNA) in good standing and current on the Minnesota Nursing Assistant Registry.
Dependable, punctual, trustworthy, and self-motivated.
Genuine concern for the well-being of clients.
Able to work independently and follow care plans, policies, and procedures.
Strong communication and interpersonal skills.
Able to work cooperatively with clients, families, coworkers, and supervisors.
Dependable transportation to clients' homes.
Able to safely perform required physical tasks and use proper body mechanics.
Responsibilities
Review and follow each client's written care plan.
Provide personal care, including bathing, grooming, oral hygiene, skin care, and toileting.
Instruct or assist clients with transfers and walking.
Perform light housekeeping tasks.
Observe and report changes in client condition or safety concerns to the RN.
Immediately report any vulnerable adult situation (physical/verbal abuse, neglect, or unsafe living conditions).
Accurately complete documentation after each client visit.
Follow infection control, safety, and home care policies and procedures.
Participate in staff meetings, training, and care planning.
Maintain professional appearance and conduct.
Perform other duties as assigned.
