Pharmacist - Drug Use Evaluation

Promote the safe, effective, evidence-based, and cost-conscious use of medications across the organization through medication-use evaluation, stewardship, and optimization initiatives.
Serve as a clinical resource for medication information across the organization, ensuring healthcare teams have access to accurate, current, and evidence-based drug information to support safe and effective medication therapy.
Improve patient outcomes by evaluating medication-use processes, prescribing practices, and clinical outcomes to identify opportunities for improvement.
Provide clinical expertise to support formulary management, medication-use policy development, and evidence-informed decision-making.
Advance continuous quality improvement by analyzing medication-use patterns, translating emerging evidence into practice, and implementing best practices in pharmacotherapy.
Collaborate with interdisciplinary teams to enhance medication safety, optimize resource utilization, and support high-quality, patient-centered care
Support compliance with regulatory, accreditation, and organizational standards while contributing to strategic initiatives related to medication management and healthcare quality.
Medication Use Evaluation & Clinical Optimization
Develop, implement, and continuously improve the organization's Drug Use Evaluation (DUE) Program in collaboration with the Clinical Pharmacy Manager.
Design, lead, and evaluate DUE initiatives to assess the appropriateness, effectiveness, and safety of medication therapy.
Conduct medication-use reviews and evaluate prescribing practices against evidence-based guidelines, clinical standards, and formulary requirements.
Identify opportunities to improve medication-use processes and develop evidence-based recommendations.
Collaborate with clinicians and stakeholders to implement approved recommendations and support practice change.
Monitor the impact of DUE initiatives and evaluate outcomes to ensure sustained improvement.
Respond to clinical inquiries from healthcare teams regarding medication selection, dosage, interactions, side effects, and evidence-based therapy options
Medication Safety, Quality Improvement & Compliance
Identify, assess, and mitigate medication-related risks, prescribing variation, medication waste, and medication expiry.
Support medication shortage, backorder, recall, adverse drug reaction reporting and risk-management processes.
Participate in the development, review, and implementation of medication-use policies, procedures, standards, and clinical pathways.
Provide medication management expertise to support compliance with accreditation, regulatory, and organizational requirements.
Collaborate with interdisciplinary teams to strengthen safe medication-use practices and continuous quality improvement
Data Analytics
Analyze, interpret, and report medication-utilization data, quality indicators, and clinical outcomes to support organizational decision-making.
Collaborate on the development reports, dashboards, and presentations that communicate medication-use trends, outcomes, and opportunities for improvement.
Support evidence-informed formulary management, including medication additions, restrictions, therapeutic interchange initiatives, and formulary deletions.
Evaluate the clinical, operational, and financial impact of formulary and medication-use initiatives.
Lead and support medication-use research, program evaluation, and special projects related to medication optimization.
Leadership, Governance & Interprofessional Collaboration
Serve as Secretary and clinical resource to the Pharmacy & Therapeutics Committee, supporting agenda development, documentation, action tracking, and follow-up activities.
Present DUE findings, recommendations, and outcome reports to committees, leadership teams, and other stakeholders.
Collaborate with physicians, pharmacists, nurses, and healthcare leaders to support implementation of medication-use improvements.
Provide education, consultation, and knowledge translation activities related to medication-use evaluations, formulary decisions, and best practices.
Support change management initiatives that promote adoption and sustainability of evidence-based medication-use practices.
BScPhm, PharmD or equivalent
Post-graduate training is preferred.
Licensed under Part A of the Ontario College of Pharmacists Register required in good standing
Active membership in the Canadian Society of Hospital Pharmacists (CSHP) is preferred.
Successful completion of a Pharmacy Residency Program accredited by the Canadian Hospital Pharmacy Residency Board, or equivalent experience is preferred
Minimum of 5 years of relevant hospital clinical experience including progressive professional practice, education and leadership experience required.
Experience and familiarity with professional standards of practice and with applicable legislation including but not limited to, Regulated Health Professions Act and profession-specific acts
Experience in leading or coordinating pharmacy medication related projects or initiatives
Completion of leadership training is an asset
