Schedule Details/Additional Information:
Monday-Friday 8am-5pm (weekends as needed). This position requires local travel within assigned service area and occasional travel across enterprise service areas; and may involve exposure to weather and road conditions.
Pay Range:
$54.90 - $82.35
Major Responsibilities:
Partner with local facility leaders, clinical teams, operational stakeholders, and enterprise Community Health & Impact leaders to align local community health priorities with broader organizational strategies, service line priorities, and population health goals.
Lead the Community Health Needs Assessment cycle and related Implementation Strategies and governance responsibilities for assigned markets or facilities, ensuring identified priorities are translated into actionable plans that reflect local needs, division/facility priorities, and enterprise Community Health & Impact direction.
Develop, implement, and refine community health programs that address priority social drivers of health and support measurable improvements in social and clinical outcomes for patients, communities, and targeted populations.
Establish performance measures, evaluation approaches, and reporting processes to assess program effectiveness, demonstrate impact, and inform continuous improvement of community health strategies and investments.
Source, assess, and advance local community projects for potential low-interest loan investment, including coordination with internal partners to evaluate alignment, readiness, impact potential, and strategic fit.
Support Community Engagement strategies to build and maintain productive relationships with community-based organizations, public agencies, civic leaders, philanthropic partners, and other local stakeholders to support shared priorities and strengthen Advocate Health’s role as a trusted community partner.
Oversee timely and accurate community benefit data entry, documentation, and validation processes to support internal reporting, regulatory expectations, and consistent enterprise standards.
Translate community data, partner insights, and implementation progress into clear recommendations, presentations, and updates for local leaders, enterprise stakeholders, and community partners.
Support cross-functional collaboration and project execution by convening internal and external partners, monitoring progress against agreed-upon goals, and addressing barriers that may affect successful implementation of community health initiatives.
Licensure, Registration, and/or Certification Required:
N/A
Education Required:
Bachelors degree in Public Health, Public Administration, Community Planning, Health Administration or other relevant field.
Experience Required:
7 years of experience addressing and improving the social drivers of health, community health strategy, population health initiatives, or related areas.
Knowledge, Skills & Abilities Required:
Experience with building strategy to improve health in underserved communities.
Ability to drive clarity, alignment, and decision support across a complex organization, including senior-level executives.
Knowledge of current and emerging best practices in large health systems addressing community need, social drivers of health and health disparities.
Knowledge of public health data collection and analysis.
Familiarity with Community Benefit Reporting approaches and standards.
Experience addressing social drivers of health, specifically with coalitions and community-based partners.
Strong analytical and writing skills.
Demonstrated ability to build, convene, and align cross-functional teams and working groups toward shared goals, with capacity to lead through influence and drive accountability across a matrixed organization.
Ability to develop and manage projects within complex community environments.

Operates equipment necessary to perform job functions.
This position may function virtually and requires the ability to work from a non-Advocate Health location.
Preferred Education:
Masters degree in Public Health, Public Administration, Community Planning, Health Administration or other relevant field.

Strong analytical and writing skills.
Knowledge of current and emerging best practices in large health systems addressing non-clinical drivers of health.
Understanding of how housing stability, food security, economic opportunity, and other structural factors influence health outcomes and health access.
Experience developing strategy, partnership approaches, or investment opportunities across sectors and stakeholders.
Ability to drive
