Physical Therapist (PT) – Home Health | Buffalo, NY | Full-Time & Part-Time | 3 Openings.
Stern at Home Therapy is expanding throughout the Buffalo, NY, area and is hiring three Physical Therapists (PTs) to support our continued growth.
We offer opportunities across a broad territory from North Buffalo through the city and down into the Southtowns, allowing clinicians to build a caseload closer to home rather than covering the entire region. The Southtowns generally include communities such as West Seneca, Orchard Park, Hamburg, East Aurora, and surrounding areas.
We are hiring for Full-Time and Part-Time, making this a strong opportunity whether you're building a primary caseload or adding flexible visits around an existing position.
What You'll Do:
As a Physical Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes.
You'll have the clinical autonomy to develop individualized plans of care while our administrative team supports you with referrals, scheduling, authorizations, and patient coordination.
With three openings available, we're building coverage across different sections of the Buffalo market so clinicians can develop geographically concentrated caseloads.
Responsibilities include:
Perform physical therapy evaluations and develop individualized plans of care.
Provide one-on-one physical therapy treatments in patients' homes.
Help patients improve strength, balance, mobility, endurance, and functional independence.
Provide gait training, transfer training, fall-prevention strategies, and home exercise programs.
Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
Complete accurate and timely clinical documentation.
Communicate with physicians, caregivers, families, and the interdisciplinary care team.
Coordinate with our administrative team to develop a caseload within your preferred Buffalo-area territory.
Compensation: Full-Time – Minimum 16 Patients.
W-2 Tiered Per-Visit Option:
$45–$60 per follow-up visit.
$65 per 4-unit evaluation.
Your per-visit rate increases as your monthly patient volume grows.
1099 Tiered Per-Visit Option:
$60–$70 per follow-up visit.
$75 per evaluation.
Your per-visit rate increases as your monthly patient volume grows.
Part-Time – Minimum 10 Patients.
W-2:
$55 per follow-up visit.
$65 per 4-unit evaluation.
1099:
$62 per follow-up visit.
$75 per evaluation.
Full-Time W-2 Benefits:
401(k) with annual match – employer match starts at 1.5% and increases up to 7%.
Paid Time Off – 2 weeks after 6 months and a third week after 9 months.
5 paid holidays – effective immediately, with the flexibility to choose which holidays you take.
Student loan repayment assistance – eligible after your first year, starting at $125 per month.
$500 annual CEU stipend.
$250 commuter stipend for full-time visits.
Childcare assistance program in development.
Equipment provided.
Part-Time W-2 Benefits:
401(k) with annual match.

Prorated commuter stipend based on patient volume, up to $175 per month at 10 patients.

Qualifications:
Active Physical Therapist license in New York required.
Graduate of an accredited Physical Therapy program.
Reliable transportation required.
Strong clinical, communication, documentation, and time-management skills.
Ability to work independently in a home-based setting.
Home health or geriatric experience is preferred but not required.
