Home-Based Physical Therapist

Role Overview: The OP Home-Based Physical Therapist provides comprehensive physical therapy services, including assessments, treatment planning, and interventions, adhering to professional and ethical standards. They contribute to program development, quality improvement, and interdisciplinary problem solving.
Key Responsibilities
Conduct evaluations and deliver treatments such as muscle strengthening, gait training, ADL, and equipment management for patients of all ages.
Create and update individualized treatment plans with clear goals, monitoring patient progress.
Complete accurate and timely clinical documentation per guidelines.
Educate patients and caregivers on activities between sessions.
Communicate regularly with healthcare team members and referring physicians regarding patient progress.
Manage appointment scheduling efficiently, including travel routes.
Maintain department policies, safety, and quality standards.
Ensure compliance with insurance authorizations and communicate concerns to management.
Handle portable therapy equipment and maintain departmental supplies and equipment.
Perform additional duties as assigned.
Qualifications
High School Diploma or GED required; Doctor of Physical Therapy (DPT) after 2015 required.
Licensed Physical Therapist and valid driver’s license essential.
Auto liability insurance and BLS certification within 90 days.
Must have reliable transportation for the role.
