ContinuumHospice is currently hiring a Per Diem Compassionate Resource Nurse (RN) to provide patient care in and around Sonoma & Marin county! Our Resource Nurses (RN) are responsible for assuring that the Hospice programs are effectively represented to patients, families, and others who have an interest in hospice through pre-assessments, information sharing and/or admissions in a Patient's home or in a health care facility. This position works with the intake team, referring physician, and other referral sources to promote the hospice philosophy of care.
