Baptist Health is seeking a dedicated and compassionate Registered Nurse to join our Case Management team. This is a Full Time day shift opportunity and plays a key role in supporting patients throughout their hospital stay by ensuring coordinated, high-quality care.
Registered Nurse, Case Management, Job Responsibilities
Provide comprehensive care coordination for hospitalized patients.
Conduct high-risk screenings and assessments to identify patient needs.
Arrange and document post-acute care services, ensuring smooth transitions of care.
Collaborate with a multidisciplinary healthcare team to support optimal patient outcomes.
Experience Requirements:
1 year case management experience
Education & Credential Requirements:
Associate of Science in Nursing required
Bachelor of Science in Nursing preferred
Basic Life Support (BLS) required
FL Licensed Registered Nurse required
