Hospice Liaison

Presbyterian Homes & Services is seeking an Hospice Liaison for its team. The Hospice Liaison advances PHS’ mission by building trusted relationships and providing non-clinical hospice education to PHS senior living communities, residents, families, and referral partners. Through participation in community transition and at-risk huddles, proactive outreach, and collaboration with community and clinical leaders, this field-based role supports earlier identification of residents who may benefit from hospice and helps ensure timely, informed decisions that respect each individual’s freedom of choice. The Hospice Liaison reports to the Director of Business Development.
