We’re looking to grow our team with a Virtual Care Coordinator who will report to our Manager of Virtual Care Operations. In this role, you’ll support patients and clinicians nationwide by coordinating care delivered by our Interdisciplinary Care Team, including Health Coaching and Registered Dietitians. You will help to triage and resolve non-clinical patient needs, and ensure a seamless care experience in a fully remote environment.
This role is ideal for someone with prior virtual care experience who is comfortable navigating EMRs, managing multiple priorities, and working in a fast-paced, digital-first health tech startup.
Responsibilities:
- Partner with cross-functional teams, including Call Center, Patient Access, and Billing, to coordinate a patient’s health coaching and/or nutrition journey.
- Outreach to patients to help them navigate their care journey, including insurance and payment considerations, evaluating service options, and appointment scheduling.
- Manage inbound patient messages, escalating clinical questions and concerns to the appropriate clinician.
- Own case-by-case service recovery efforts, including developing and executing action plans and coordinating patient communication to resolve concerns.
- Manage Remote Patient Monitoring (RPM) program enrollment workflows and device logistics in partnership with internal teams and external vendors.
- Provide real-time administrative support and issue resolution to clinicians and patients during virtual visits, including technical troubleshooting as needed.
- Collaborate with the Manager of Virtual Care Operations and cross-functional team members to identify opportunities to improve workflows and operational processes.
- Deliver a warm, inclusive, and professional patient experience aligned with knownwell’s values.
Qualifications:
- High school diploma or equivalent
- 2+ years of experience in a virtual care, patient coordination, or Medical Assistant role
- Familiarity with virtual care workflows in a primary care or chronic care setting
- Prior experience supporting care in a multi-state, remote care delivery model
- Strong communication and organizational skills
- Comfort with remote tools, secure messaging, EMR platforms (Athena preferred)
- Tech-savvy, adaptable, and comfortable operating independently in a remote setting
