Responsibilities include:
Assisting with personal care (bathing, grooming, dressing, toileting, feeding)
Performing light housekeeping and grocery shopping
Preparing meals and ensuring proper nutrition
Monitoring vital signs (heart rate, blood pressure, temperature)
Assisting with basic health needs
Scheduling medical appointments and providing transportation
Offering companionship and engaging in activities
Keeping accurate records of patient care and health status
Salary: Starting at $19.65 per hour + benefits

Requirements :
Must have NYS HHA Certificate
Must be able to work at least three shifts per week, including weekends
Must be available to take 4-6 hour shifts in addition to longer 8-12 hour cases
Must be able to provide annual physical, PPD, MMR titres & other NYS DOH Required compliance documentation
Must be able to pass NYS Department of Health Criminal History Record Check
