SWAT Supervisor – Social Worker

Presbyterian Homes & Services is seeking a SWAT Supervisor - Social Worker to join its team. The SWAT Supervisor provides direct supervision and oversight of visiting Social Workers and Chaplains. This position is responsible for case assignments, promoting strong collaboration with interdisciplinary staff, and maintaining effective communication with clients, families, and referral sources. The SWAT Supervisor serves as a resource and educator for field staff. The SWAT Supervisor (Social Worker) may, as needed, allocate a portion of their time providing medical social services for hospice clients consistent with regulations and established best practices.
