PRN Home Health & Therapy is seeking a dedicated Occupational Therapist to join our growing team. This full-time position provides the freedom to manage your own schedule while making a lasting impact on patients throughout Pulaski and surrounding areas.
In this role, you'll primarily provide care in a home health setting, with occasional visits to skilled nursing facilities. Whether you're an experienced clinician or a new graduate eager to begin a rewarding career in home health, this position offers an excellent opportunity to grow and succeed.
Key Responsibilities
Work independently while maintaining strong communication with the clinical team.
Review and respond to patient referrals in a timely manner.
Collaborate with patients, caregivers, and interdisciplinary team members to develop individualized care plans.
Create and implement treatment plans with measurable, patient-centered functional goals.
Educate patients and caregivers to support successful outcomes and maximize independence.
Provide guidance, direction, and supervision to Certified Occupational Therapy Assistants (COTAs).
Assess and recommend durable medical equipment (DME), including wheelchairs, adaptive equipment, and other assistive devices.
Partner with healthcare professionals to ensure coordinated, high-quality patient care.
Perform additional duties as assigned.
