Job Title: Peer Leader / Workshop Facilitator
Reports to: Chief Operating Officer (COO)
Location: Washington, DC
Position Type: Contractor (1099)
Date: October 2026
General Summary
The DC Living Well Program provides evidence-based workshops that help adults living with chronic health conditions and their caregivers build skills to manage their health and improve their quality of life. Participants learn practical approaches to goal setting, problem-solving, managing symptoms, healthy eating, physical activity, and communicating with healthcare providers. WellCentric Health is recruiting Peer Leaders, Master Trainers, and Senior Master Trainers to expand workshop offerings in Washington, DC. Facilitators create a supportive learning environment, encourage participation, and help community members put self-management skills into practice. Facilitators will report to the WellCentric Health program team.
Facilitators work alongside another trained leader to deliver a six-week workshop series in one or more of the following programs, depending on completed training:
Chronic Disease Self-Management Program (CDSMP)
Diabetes Self-Management Program (DSMP)
Cancer: Thriving and Surviving (CTS)
Facilitators report to the WellCentric Health program team.
Compensation is based on trainer designation:
Peer Leader: $750 (Pay per Completed Six-Week Series)
Master Trainer: $ 1,000 (Pay per Completed Six-Week Series)
Senior Master Trainer: $1,200 (Pay per Completed Six-Week Series)
NB: These amounts cover the entire six-week workshop series, rather than each individual session. Payment is issued after the full series is completed. When applying, include your trainer designation (Peer Leader, Master Trainer, or Senior Master), completed SMRC training, and the programs you are qualified to facilitate.
Applicants interested in becoming Peer Leaders should indicate their willingness to complete the required training.

Prepare workshop materials at least two days before each series begins.
Arrive at least 30 minutes before in-person sessions, begin and end sessions on time, and return the classroom to its original condition.
Encourage participant engagement and communicate reminders and upcoming workshop topics between sessions.
Maintain accurate attendance records, participant information, and evaluation forms.
Submit required workshop documentation within two weeks of series completion.
Protect participant confidentiality and follow applicable data security requirements.
Communicate workshop progress, challenges, and participant concerns to the program team.
Provide at least 24 hours’ notice of an absence whenever possible and work with the program team to identify qualified substitute coverage.

Current training in an approved SMRC program, or eligibility and willingness to successfully complete required Peer Leader training before facilitating.
Strong communication, organizational, interpersonal, and group facilitation skills.
Ability to follow a structured curriculum and work collaboratively with a co-leader, participants, community partners, and program staff.
Reliable transportation to assigned workshop locations in Washington, DC.
Scheduling flexibility and availability to facilitate the full six-week series for accepted assignments.
Commitment to maintaining active facilitator status and meeting applicable SMRC and program facilitation requirements.
Willingness to sign and comply with the Peer Leader Memorandum of Understanding.
Applicants seeking Master Trainer or Senior Master Trainer assignments must provide documentation supporting their applicable trainer designation.
Preferred Experience
Experience in community education, group facilitation, or supporting adults with chronic health conditions is preferred. Personal experience managing a chronic condition or caring for someone who does, along with familiarity with Washington, DC communities, is also valued.
