Founding Engineer (AI Voice + Backend)

ClinicFlow is a voice AI receptionist for orthopedic, spine and surgical practices. It answers every patient call 24/7, books and reschedules appointments directly in the practice's EMR, applies the escalation rules the surgeons set, and texts the on-call surgeon when a call is urgent. It is live and answering real patient calls today.
We are two co-founders: Comron Saifi, a practicing spine surgeon, and Neel Patel. You would be the engineer who owns the system end to end.
WHAT YOU'LL OWN
• The voice agents in production (Python, LiveKit Agents, speech-to-text and text-to-speech vendors, LLM prompts and evals)
• The scheduling backend and EMR integrations (Java/Spring Boot, FHIR and HL7, eClinicalWorks, athenahealth, ModMed)
• The practice dashboard and API (TypeScript, Express, Next.js)
• AWS infrastructure, CI/CD and releases (ECS, Aurora Postgres, DynamoDB, Terraform, GitHub Actions)
• Security and HIPAA hygiene for a product that touches patient data
• Working with AI coding agents every day. We use Claude Code heavily, and you'll direct agents, not just write code.
YOU MIGHT BE A FIT IF YOU
• Have shipped software real people used, and can show it
• Are strong in at least one of Java, TypeScript or Python and happy to work across all three
• Want production ownership: you'd rather fix it at 11 p.m. than file a ticket
• Write clearly and answer fast
• Are early in your career and trained at a top CS/EE program (MIT, Stanford, CMU, Berkeley, Waterloo and the like), or have equivalent proof: competitions, open source, a startup you built
Bonus: realtime audio or telephony (LiveKit, Twilio, WebRTC), LLM agents and evals, FHIR/HL7, healthcare.
COMPENSATION
• Full-time: $6,000-$8,000 per month, plus 1%-5% equity (4-year vest, 1-year cliff)
• Part-time (20+ hours a week) is welcome, with cash pro-rated
• You can trade cash for more equity
LOCATION
Remote, US or Canada, with at least 4 hours a day overlapping US Central time.
