$5,000 Sign-On Bonus | Full-Time & PRN | $65,000–$90,000/year

We’re looking for compassionate, skilled RNs / Registered Nurses to join our team as Home Health Case Managers serving Jefferson City and surrounding Missouri communities.

What You’ll Do
As a Home Health Case Manager RN, you will provide skilled nursing care while coordinating a patient's overall plan of care.
Your responsibilities will include:
Manage an assigned home health nursing caseload
Provide skilled nursing visits, evaluations and starts of care
Supervise LPN visits and coordinate care among healthcare providers
Complete comprehensive nursing assessments and develop individualized plans of care
Communicate with physicians to obtain orders and establish diagnosis-driven plans of care
Coordinate with insurance companies and other healthcare professionals
Educate patients and families about conditions including CHF, COPD, diabetes, wound care and medication management
Collaborate with Physical Therapy, Occupational Therapy and Speech Therapy
Provide hands-on patient care in patients' homes
Use clinical judgment, physical assessment skills and diagnostic information to support quality patient outcomes
What We’re Looking For
Missouri RN license or Compact RN license
Valid driver’s license
Reliable vehicle with current auto insurance
Home health nursing experience preferred
Critical care experience preferred
Strong assessment, communication and clinical decision-making skills
Ability to work independently while collaborating with an interdisciplinary care team
