About Total Life
Total Life is a telehealth company dedicated to older adults. We provide behavioral health, geriatric care, and longevity services to adults 65+, billed through Medicare, Railroad Medicare, and major commercial insurers. Our network includes 200+ credentialed providers serving patients across 49 states.
We're launching a new Longevity Program focused on healthy aging — comprehensive geriatric assessment, chronic disease management, cognitive health, and weight management including GLP-1 therapy.
The role
You'll deliver initial longevity assessments and ongoing medical follow-ups to older adults via telehealth. This includes comprehensive geriatric assessment, cognitive screening, medication review, advance care planning, and management of chronic conditions. You'll work alongside our CMO, a board-certified geriatrician, with full training and clinical support provided.
What you'll do
Conduct comprehensive initial longevity assessments (60 min) covering functional status, cognition, nutrition, mobility, and fall risk
Provide medical follow-up visits for chronic disease management and ongoing care
Perform cognitive screening and staging; escalate for further assessment where indicated
Prescribe and manage GLP-1 therapy for appropriate patients
Conduct advance care planning conversations
Document in our EHR to support accurate Medicare billing
Requirements
Requirements
Active, unrestricted NP license in Texas
Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with geriatric focus, training, or demonstrated interest in aging
Active DEA registration
Experience with Medicare documentation and E/M coding
Comfort with telehealth and EHR documentation
Reliable high-speed internet and a private, HIPAA-compliant workspace
Nice to have
Experience with comprehensive geriatric assessment
Familiarity with cognitive screening instruments (Mini-Cog, MoCA)
GLP-1 prescribing experience
Multi-state licensure
