What You'll Do & Bring
As a Registered Nurse (RN), you'll provide skilled nursing care to patients in their homes, helping them manage acute and chronic conditions, recover from illness or injury, and maintain the highest possible level of independence. You will work closely with physicians, caregivers, and other healthcare professionals to develop and implement individualized care plans that promote positive health outcomes.
Key Responsibilities
Conduct comprehensive patient assessments, including physical, emotional, and environmental evaluations.
Develop, implement, and periodically revise individualized Plans of Care in collaboration with physicians and the interdisciplinary care team.
Provide skilled nursing services, including wound care, medication management, IV therapy, catheter care, and disease-specific interventions as ordered by the physician.
Monitor patient conditions and identify changes that may require modifications to the care plan or physician intervention.
Administer medications and treatments according to physician orders and agency policies.
Educate patients, families, and caregivers regarding diagnoses, medications, disease management, safety practices, and treatment plans.
Coordinate care with physicians, therapists, social workers, home health aides, and other healthcare professionals.
Ensure timely communication with physicians regarding patient status, treatment effectiveness, and care needs.
Maintain accurate, complete, and timely clinical documentation in compliance with agency policies and state and federal regulations.
Participate in case conferences and interdisciplinary team meetings.
Promote patient safety and infection prevention practices during all visits.
Supervise and delegate appropriate patient care activities to Licensed Vocational Nurses (LVNs), Home Health Aides, and other support staff as applicable.
Participate in required continuing education, quality improvement initiatives, and professional development activities.
What You Bring
Graduate of an accredited nursing program.
Current Texas Registered Nurse (RN) license in good standing.
Minimum one (1) year of nursing experience required.
Home health experience strongly preferred.
Knowledge of home health regulations, standards of practice, and Medicare guidelines preferred.
Strong clinical assessment and critical-thinking skills.
Excellent communication and patient education abilities.
Strong organizational and documentation skills.
Ability to work independently while maintaining strong collaboration with the care team.
Compassionate, patient-centered approach to care.
Reliable transportation and ability to travel throughout assigned service areas.
Current CPR or Basic Life Support (BLS) certification.
Bilingual English/Spanish preferred.
Work Location & Travel Requirements
Home office location: MyCHN Home Health Services 4005 Technology Drive, Angleton, TX 77515
This position requires regular travel to patient homes and other assigned locations throughout Brazoria, Galveston, and Harris Counties.
