Job /Roles Summary
The Council HIV/TB Sustainability Coordinator is a self-motivated and results-oriented professional responsible for providing council-level oversight and coordination of the Regional Acceleration Strategy (RAS) and ensuring effective implementation of project-supported activities within the assigned councils. He/she will work closely with the Council Health Management Team (CHMT), facility in-charges, health care providers, Community Health Workers (CHWs), and community case management teams to ensure comprehensive and integrated council facility and community level planning, implementation, monitoring, documentation and reporting of supported interventions in the context of sustainability. The Council HIV/TB Sustainability Coordinator will coordinate implementation across facilities and communities to ensure that project interventions are delivered with quality, efficiency, accountability, and measurable results.
The Council HIV/TB Sustainability Coordinator will have overall responsibility and accountability for council-level project results, including planning, implementation, monitoring, documentation, performance review, and timely reporting at council, regional, and project levels. s/he will use routine program data to identify performance gaps, facilitate timely course corrective actions, and ensure that implementation remains aligned with council priorities, project targets, and government strategies.
Reporting to the Zonal Associate Director HSS/ Program Sustainability and Senior Manager HIV/TB Sustainability -(SMHTS) - RAS, the Coordinator will serve as the Foundation’s key council-level representative and relationship manager, working closely with CHMTs, sub-partners, collaborating partners, Faith-Based Organizations (FBOs), and other stakeholders within the council. The Coordinator will lead and coordinate a multidisciplinary council-level case management team, comprising technical advisors and officers, M&E and operations staff, council-owned health care workers, project-supported human resources for health, FBO staff, CHWs, and community volunteers supporting project-funded interventions. The Council HIV/TB Sustainability Coordinator will work closely and collaboratively with the CHMTs to strengthen a sustained HIV/TB response and promote long-term program sustainability. This will include jointly planning and implementing council-level RAS activities, conducting routine monitoring and evaluation of project performance through weekly, monthly, and quarterly reviews, and using program data to identify performance gaps and emerging challenges. The Coordinator will facilitate timely corrective actions and course correction to prevent underperformance, address implementation bottlenecks, and maintain the quality, continuity, and sustainability of supported HIV/TB services.
Key Responsibilities – Council-Level
Provide overall council-level leadership and coordination to achieve project results, including organizing and supporting an effective RAS case management team structure and workflow across council facilities and communities.
Lead council-level RAS work planning, implementation, monitoring, documentation, and reporting, ensuring activities are implemented in line with approved plans, priorities, and available resources.
Serve as the council-level project representative and relationship manager, coordinating effectively with sub-partners, collaborating partners, implementing partners, and other key stakeholders within the supported council.
Work closely with CHMTs to ensure RAS priorities are reflected in the Council Comprehensive Health Plan (CCHP) and aligned with supported project strategies, priorities and activities fully integrated within council plans, targets, and government priorities to ensure sustainability beyond life of projects.
Ensure well-coordinated and functional roving and case management teams across councils and facilities, with team activities guided by data-driven performance priorities, identified service delivery gaps, and available project technical and operational resources.
Coordinate with CHMTs and facility teams to facilitate implementation of planned activities, routinely review progress against targets, identify implementation gaps, and provide timely, actionable feedback and support for continuous improvement.
Maintain overall responsibility and accountability for council-level project performance and results, including site-level and council-level performance against agreed HIV/TB targets and quality standards.
Provide oversight to ensure that relevant government and donor SOPs, guidelines, policies, and technical standards are available, understood, and consistently adhered to throughout project implementation, monitoring, and reporting.
Ensure quality, accuracy, completeness, and timely submission of council-level weekly, monthly, quarterly, and annual reports and inputs, including achievements, successes, lessons learned, implementation challenges, and proposed corrective actions.
Use routine performance data and monitoring findings to facilitate immediate course correction and performance improvement, preventing underperformance and maintaining the quality, continuity, and sustainability of supported HIV/TB services.
Promote local ownership and sustainability by working with CHMTs and facility teams to integrate project-supported interventions into routine council planning and budgeting.
To be successful, you will have:
Graduate or postgraduate degree in Medicine, Nursing, Public health or Project Management
Over 6 years of experience supporting and or leading HIV/TB integrated services including case identification and treatment.
Thorough understanding of latest Integrated HIV, STI and Hepatitis Management Guidelines and existing tools and Standard Operating procedures for patient’s management.
Proven experience in staff management, including expertise in coaching, mentorship and capacity building
Ability to advocate, provide technical assistance and facilitate domestic resources mobilization activities and monitor to ensure sustainability supported health services and interventions
Previous experience in quality assurance training, mentoring, motivating teams, developing to develop councils and facility level indicators towards services sustainability
Proven ability to analyse program data and develop evidence-based strategies
Strong verbal and written communication skills in English
Excellent communication skills and experience working with multicultural teams
Strong attention to detail, ability to meet deadlines, and collaborative team spirit
