What You Will Do
Identify, assess, and plan care strategies for patients, ensuring their health care needs are met.
Facilitate care coordination and advocate for patients and their families.
Provide discharge planning services to support safe transitions to the next level of care.
Manage and address the needs of patients with critical diagnoses or events.
Collaborate with the healthcare team to promote cost-effective and high-quality outcomes.
Utilize community resources to support diverse patient populations.

What You Will Need
Required Qualifications
Bachelor's degree in nursing or Master's degree in Social Work.
Minimum 3 years of clinical experience.
Excellent verbal and written communication skills.
Strong customer service and interpersonal skills.
Proficiency in problem-solving, analytics, and decision-making.
Computer literacy and knowledge of healthcare systems.
Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid Michigan license.
Preferred Qualifications
Experience in discharge planning, home health care, or managed care.
Certification in Case Management (CCM) or Accredited Case Manager (ACM).
