ABOUT
Moore Consulting, LLC operates CareMoore, a community health company working at both ends of the health system to ensure policy serves people. Our Consulting Practice partners with organizations and employers on systems-level work, while our Community Practice works directly with families through hands-on care programs. Current community programs include our Healthy Heart Initiative, a mobile outreach initiative connecting men to primary care and social supports, and our caregiver support program for families caring for someone living with dementia, working toward actively engaging 100 caregivers.
This person is responsible for successfully delivering our current programs and growing our presence and reach in the community, owning whether the work actually gets done, on time, to a high standard, without the CEO having to check. As deputy to the CEO, this role carries real growth opportunity as the organization grows.
POSITION SUMMARY
The Community Program Manager and Operations Lead is the operational leader for our community health programs, bringing attention to detail, follow-through, and high standards to ensure we deliver with excellence. This person owns execution of our current community health work end-to-end, including project management, staff and consultant management, compliance and reporting, community partnerships, data and tracking systems, and program quality, and helps grow and expand new programs over time.
WHAT SUCCESS LOOKS LIKE (FIRST 12 MONTHS)
• Both of our current DC Health work plans are on track against their approved objectives, deliverables, and completion dates, with no missed monthly or quarterly DC Health reporting deadlines.
• The CEO is no longer the person tracking day-to-day tasks, chasing deliverables, or personally reviewing every piece of outreach; she is reviewing a scorecard and getting escalations only when something is genuinely off track.
• The Healthy Heart Initiative reaches its monthly targets (≥200 residents reached, ≥50 referrals to Men's Health Navigators) and the caregiver support program reaches its goal of 100 actively engaged caregivers.
• Community Influencers and program staff are trained, deployed, and meeting their commitments, with a clear escalation path when they aren't.
• Support key partnership development (Men's Health Navigators, LinkU, DACL, Alzheimer's Association, CBOs).
• Grant compliance is audit-ready at all times: budgets tracked against approved line items, data systems current, documentation organized, with no last-minute scrambles before a site visit or report deadline.
CORE RESPONSIBILITIES
Program Delivery & Work Plan Ownership
• Own execution of our DC Health-approved work plans: track every objective, activity, and deadline; keep the CEO informed with a simple, honest status view rather than surprises.
• Run the monthly cadence: staff/consultant/partner check-ins, tracking and reporting, quarterly outreach plan revisions, etc.
• Identify when a program is off track early enough to fix it, propose a course correction, and drive it, not just report the problem.
People Management
• Directly manage the Care Navigator/Community Health Worker, Communications Lead, and program consultants; oversee recruitment, training, deployment, and performance of Community Influencers and Men's Health Navigator/referral partners.
• Set clear expectations and hold people to them. Follow-through and attention to detail are non-negotiable, and this role models that standard for everyone under it.
Compliance, Data & Reporting
• Maintain program data systems (intake/registration, Caregiver ID tracking, referral and outcome tracking, dashboards) and ensure data is accurate, current, and audit-ready.
• Own budget tracking against each grant's approved line items and flag variances before they become problems.
• Prepare and submit all required DC Health deliverables (monthly reports, quarterly revisions, year-end impact summaries) on time and to a high standard.
Community & Partnerships
• Maintain and grow referral pathways and partnerships across the DC communities we serve: health centers, CBOs, faith-based and cultural centers, senior wellness centers, and other trusted community venues.
• Represent the organization in the community when needed, and ensure Influencers and staff are doing the same with consistency and quality.
Organizational Support
• Act as the CEO's deputy on operational matters.
• Build the systems, structure, processes, and documentation (checklists, tracking tools, regular check-ins) that this growing organization needs to ensure the team runs well and delivers high-quality results.
WHAT WE'RE LOOKING FOR
• 5+ years managing community health, social services, or public health programs. Grant-funded and government program experience strongly preferred.
• Demonstrated ownership of a program end-to-end: you can point to something you ran where the deliverables, budget, and reporting were yours and they came in on time and on standard.
• A genuine self-starter who does not need to be managed closely: someone who sees what needs to happen and does it, then tells you it's done.
• High standards for accuracy and follow-through, with the discipline to build and maintain the tracking systems that prove it.
• Experience managing people, whether staff, contractors, or volunteers, including holding them accountable to deadlines and quality.
• Comfortable in both community settings (barbershops, churches, senior centers, community events) and in compliance-heavy administrative work. This role moves between both in the same week.
• Bachelor's degree required; Master's in public health, social work, or a related field preferred. Experience can substitute for some of the above where it demonstrates equivalent capability.
• Lived or professional familiarity with the communities served, whether men's heart health, dementia/caregiving, or DC communities specifically, is a strong plus, not a requirement.
