Ready to make a real impact? Here's what you'll be doing in this exciting role:
See patients of all ages – from newborns to our most seasoned patients – and tackle whatever health challenges walk through the door
Be the acute care hero – manage urgent health issues across all age groups with skill and confidence
Stabilize and coordinate – work your magic on uncontrolled chronic conditions to get patients stable, then connect them with their primary care team for ongoing management
Flex your preventative care muscles – jump on opportunities to help patients stay healthy and prevent future problems
Fine-tune chronic care management – make thoughtful adjustments to treatment plans as needed, always keeping the patient's best outcome in mind
Own your patient load – expect to see 30-40 patients per day once you hit full capacity (don't worry—you'll have 3-6 months to ramp up!)
Be a team player – cover at our Same Day Clinic locations as needed. You'll have a primary home base at either 3235 E Michigan Ave, Jackson, MI 49202 or 1401 W North St, Jackson, MI 49202, where you'll spend most of your time, with flexibility to support our other clinic locations
Work at the top of your license – enjoy high-level autonomy with the support and collaboration of an incredible team of APPs and physicians
Schedule: Full-time, 40 hours per week with no weekend shifts. Optional on-call weeks available with extra work shift pay for those who want to expand their impact!
Required Qualifications
Master's degree in nursing (NP) or equivalent graduate degree (PA)
Current NP/PA license in Michigan
Current RN license (for Nurse Practitioner candidates)
BLS certification
ACLS/ATLS/PALS provider certification as required by department
Active DEA registration with Controlled Substance License (CSL)
Ability to provide care across all age groups (birth to death)
Collaborative practice style with comfort working autonomously within a team environment
Preferred Qualifications
1+ years of primary care experience
Proficiency in performing clinical procedures
Knowledge of quality metrics and performance-based care models
Strong patient satisfaction scores (Press Ganey)
