Case Management Assistant

Overview:
Monday - Friday, 8am - 5pm, onsite 40 hours/week

Job Description Summary:
Supports safe, timely, and efficient patient discharges and care coordination by facilitating communication, scheduling, and collaboration among patients, families, and multidisciplinary care teams.
Job Description:
Essential Functions:
Assists with the scheduling of various inpatient and outpatient services, including tests, procedures, and follow-up appointments.
Communicates with patients and families regarding admissions and appointments and connects them with appropriate resources.
Collaborates with multidisciplinary teams to ensure cohesive care planning and effective communication throughout the discharge process.
Obtains required information from patients, caregivers, healthcare providers, and other external teams.
Assists with completing required documentation such as FMLA forms and obtaining payer authorizations for medications, equipment, and services related to post-discharge care.
Monitors related metrics and assists with data gathering to support process and patient care improvements.
Ensures compliance with regulatory and organizational privacy and safety standards.
Education Requirement:
Associate's Degree, or equivalent education and experience, required.
Skills:
Knowledge of current medical terminology.
Knowledge of payer negotiations and authorization requirements.
Excellent communication and interpersonal skills.
Able to work in a fast-paced clinical environment and manage multiple tasks effectively.
Demonstrated effectiveness in working with multi-disciplinary teams.
Experience:
Experience in an inpatient or outpatient clinical setting, preferred.
Experience with electronic medical records, preferred.
