Position Summary
IMPACT is an Assertive Community Treatment (ACT) program that provides team treatment, psychosocial rehabilitation and support in the community to adults with severe and persistent mental illness and a primary diagnosis of a primary psychotic disorder. Its interdisciplinary team includes Psychiatrists, Psychiatric Nurses, Social Workers, Community Mental Health Workers, Peer Support Worker, Psychiatric Resident, as well as students from different disciplines. IMPACT strives to minimize the impact of mental illness on clients’ lives, by facilitating the integration of clients into communities of their choice, and supporting them to improve the quality of their lives, in the areas of health, housing, finance, and vocation.
Duties
Directs the day-to-day operations of the IMPACT Team
Leads daily team rounds and treatment planning meetings
Coordinates treatment activities
Triages new referrals
Conducts client-centred assessments and contributes to individualized treatment plans
Creates, manages, adjusts team members’ weekly schedule, to meet clients’ needs
Participate in recruitment processes
Orient new team members to IMPACT and their specific role within the team
Acts as a senior clinician and clinical leader for the team, providing mentorship, supervision, consultation, and support for complex clinical decision-making while promoting adherence to ACT standards and best practices.
Provide client service, as needed/directed
Completion of a Master’s Degree in a health sciences field (Social Work, Psychology, Occupational Therapy, Mental Health etc)
Registration/licensure as required by professional body, e.g. Ontario College of Social Workers and Social Service Workers, College of Occupational Therapists of Ontario, etc.
Minimum of 3 years working in mental health community setting
Minimum 1 year of supervisory experience.
Strong knowledge of the ACT model and psychosocial rehabilitation principals
Strong knowledge of concurrent disorders and harm reduction approach
Strong knowledge of trauma-informed care
Training in and experience delivering individual psychotherapeutic modalities (i.e., CBT, DBT, Behaviour Activation, coping strategies for stress), considered an asset
Expertise in crisis assessment and management
Experience and ability to effectively work as a member of an interdisciplinary team in a health care setting
Excellent interpersonal skills

Excellent problem-solving skills
Knowledge of policies, legislation, programs and issues related to mental health services, social services, community services, and placement services relevant to adults
Valid driver’s license and access to a reliable vehicle is necessary
