Role Overview
As a Patient Navigator, you will play a vital role in coordinating access to health-related services for our clients. You will guide them through the healthcare system, ensuring they receive the care and resources they need. Your work will directly impact the well-being of our patients and strengthen our community ties.

What You Will Do
Identify potential clients, complete initial needs assessments, and maintain follow-up to ensure appropriate guidance to health services.
Provide informal counseling, education, and social support to clients.
Assist clients with utilizing available resources, including scheduling medical appointments and accompanying them to appointments.
Help clients complete medical program applications.
Develop and maintain strong working relationships with external and internal contacts.
Maintain accurate client records by documenting all contacts, services provided, and outcomes.
What You Will Need
Required Qualifications
Ability to identify potential clients and complete needs assessments.
Skills in providing informal counseling, education, and social support.
Experience in assisting clients with resource utilization and application completion.
Proficiency in maintaining accurate client records and documentation.
Preferred Qualifications
Experience working in a healthcare or community support role.
Familiarity with local health resources and community agencies.
