Care Coordinator, Supervisor

The Care Coordinator, Supervisor will lead a remote team responsible for helping members navigate healthcare services, connect with providers, and manage transitions of care.
This role combines team leadership, operational oversight, and service quality management to ensure members receive timely and effective support.
You will oversee daily workflows, monitor performance metrics, coach team members, and resolve escalated issues affecting access to care.
Working with providers, internal teams, and community partners, you will help remove barriers to services and strengthen continuity of care.
You will also drive process improvements, maintain documentation standards, and ensure compliance with privacy and program requirements.
