Responsibilities
Evaluate and treat patients in the home setting
Develop individualized plans of care
Provide therapeutic interventions and functional training
Educate patients and caregivers
Complete EMR documentation
Requirements
Active NJ license as a PT
Degree from an accredited PT program
Reliable transportation
Strong communication skills
