Advanced Practice Provider, GUIDE Program Job Description

Contract, 1099
Hours per week:
8-12 hours per month, with potential opportunity for increase as program census grows. Ability to choose scheduled days/hours including Saturday.
Rate:
$500-$1000 per month, dependent on how many assessments conducted.
Role Summary:
The Advanced Practice Provider (APP) is an integral role to the success of ALLWays Supportive Care’s GUIDE program, which serves Medicare beneficiaries living with dementia and their caregivers.
The APP role includes conducting cognitive-focused clinical assessments, providing symptom management and supportive care, and providing education of disease symptoms and progression. This specific role will complete initial clinical assessments for patient as they enroll into our program.
Responsibilities:
· Cognition-focused evaluation including a pertinent history and examination
· Medical decision-making capacity assessments
· Dementia staging, using standardized instruments for staging of dementia
· Medication reconciliation and review, with a focus on high-risk medications for patients living with dementia
· Screening, or referral to screening, for hearing loss
· Making prescribing or deprescribing recommendations to the beneficiary's primary care or specialty care provider
· Understand and comply with CMS GUIDE program requirements
· Collaborate with physicians and other healthcare providers to develop and implement individualized treatment plans for patients
· Educate patients and their families about disease progression and symptom management
· Maintain accurate and up-to-date medical records
· Review and approve care plans completed by GUIDE patient navigators.
Requirements:
· Nurse Practitioner or Physician Assistant
· Current license to practice as an Advanced Practice Provider in Colorado
· Active DEA license in the state of Colorado
· Active malpractice insurance
· Strong clinical assessment and diagnostic skills
· Comprehensive knowledge on various dementia diagnosis including progression, treatment and supportive care
· Excellent communication and interpersonal skills
· Ability to work collaboratively and equally as part of a multidisciplinary team
· Proficiency in electronic medical record (EMR) systems
