Clinical Pharmacist

Equity Staffing Group is currently staffing MTM Pharmacists for Telecommute/Remote Work (Must have active Pharmacist License in State you reside in).

Responsibilities:
Engage members by phone and schedule pharmacist consultations
Follow HIPAA and privacy policy at all times
Deliver clinical recommendations (gaps in care, high-risk meds, adherence, drug interactions, substitutions) under MTM and RDUR protocols
Review medication regimens for issues with dosing, effectiveness, and adverse reactions
Educate patients on medications and disease states
Document care notes clearly and concisely
Meet productivity goals and support special projects
Required Qualifications:
Current, Must have a unrestricted Pharmacist license in the state you reside in
Strong knowledge of the 5 Core Elements of MTM Services
Comfortable navigating MTM platforms and multiple databases
Intermediate Word, Outlook, and Excel skills
Shift:
Flexible availability 9:00am–8:00pm CST, Monday–Friday, including occasional weekends
Preferred Qualifications:
PharmD
Diabetes care/education certification (CDCES, BC-ADM, ASHP, or APhA)
Bilingual in medical terminology (Cantonese, Korean, Mandarin, Russian, Spanish, or Vietnamese)
Additional pharmacist license in AL, ID, KS, KY, LA, MD, MN, MO, ND, OK, OR, TN, VT, or WV
2+ years retail/community or PBM pharmacy experience
