Role Overview
As the Supervisor of Integrated Case Management, you will oversee daily case management operations, guiding a team of professionals in delivering collaborative, patient-centered care. Your role is critical in ensuring the team effectively assesses, plans, coordinates, and evaluates care to meet the comprehensive health needs of individuals and families, promoting patient safety, quality of care, and cost-effective outcomes.

What You Will Do
Oversee daily case management operations, ensuring the team delivers high-quality, patient-centered care.
Monitor and guide case management professionals in assessment, planning, facilitation, care coordination, evaluation, and advocacy.
Participate in the development of policies, procedures, and daily workflow design.
Maintain and contribute to performance improvement activities to enhance care delivery.
Foster collaborative relationships with an interdisciplinary care team to achieve optimal patient outcomes.

What You Will Need
Required Qualifications
Bachelor's degree in nursing or related professional field (e.g., social work, counseling, health education) or a Master's Degree of Social Work.
Three (3) years of clinical experience in case management, disease management, health coaching, nursing, or home health care.
Strong communication and interpersonal skills.
Strong analytical, problem-solving, and critical thinking skills.
Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
Preferred Qualifications
Certification in Case Management (CCM) from the Commission for Case Management Certification (CCMC).
