Role Overview
As a Case Manager, you¿ll be a vital part of our patient-centered care team, responsible for identifying, assessing, and coordinating care to meet the health needs of individuals and families. Your role ensures safe transitions between care levels, promotes patient safety, and optimizes health outcomes. You¿ll advocate for patients, address complex needs, and collaborate with community resources to support diverse populations.

What You Will Do
Collaborate with the care team to identify and assess patient needs, developing individualized care plans.
Coordinate services and resources to ensure a safe transition to the next level of care.
Advocate for patients and families, addressing social determinants of health and connecting them with community resources.
Evaluate care outcomes and adjust plans to promote patient safety, quality, and cost-effectiveness.
Facilitate communication between patients, families, and healthcare providers to ensure comprehensive care.

What You Will Need
Required Qualifications
Bachelor's degree in nursing or Master's degree in Social Work.
Minimum 3 years of clinical experience.
Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
Excellent verbal communication, written documentation, and interpersonal skills.
Strong problem-solving, analytical, and decision-making abilities.
Proficiency in computer skills and knowledge of healthcare systems.
Preferred Qualifications
Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC) or Accredited Case Manager (ACM) by the American Case Management Association.
Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
