Role Overview
As a Case Manager, you are a critical member of the patient-centered care team, responsible for identifying, assessing, and coordinating care to meet the health needs of individuals and families. Your role ensures safe transitions, promotes quality care, and advocates for patients, making a significant impact on their wellbeing and recovery.

What You Will Do
Collaborate with the care team to identify and assess patient needs.
Develop and implement individualized care plans to promote patient safety and quality outcomes.
Coordinate effective discharge planning to ensure a safe transition to the next level of care.
Advocate for patients and families, addressing social determinants of health and connecting them with community resources.
Evaluate care plans and outcomes, making adjustments to optimize health along the care continuum.

What You Will Need
Required Qualifications
Bachelor's degree in nursing or a Master's degree in Social Work.
Minimum 3 years of clinical experience.
Excellent verbal communication, written documentation, and interpersonal skills.
Strong problem-solving, analytical, and decision-making abilities.
Proficiency in computer skills and knowledge.
Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
Preferred Qualifications
Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC).
Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
