Role Scope
Habitat Health is looking for a Product Manager to own strategy, roadmap, and delivery outcomes across our clinical and business applications portfolio — including EHR, CRM, Call Platform, Risk Suspecting, and ancillary clinical applications
This role would be accountable for knowing the problem space deeply, deciding what gets built and in what order, and proving that what shipped actually worked. You'll run discovery with clinical, growth, and operational stakeholders, define clear requirements and success metrics, prioritize a backlog against a real prioritization framework, and partner with technical and vendor teams to deliver against it. You'll work closely with Platform, Clinical, Operations, and Growth leaders — not just to gather requirements, but to shape what the roadmap should be, defend trade-offs with data, and drive adoption once features ship.
Primary Areas of Responsibility
● Own and evolve the product roadmap across the clinical and business applications portfolio, using a structured prioritization framework to balance strategic bets against operational demand
● Lead product discovery — engaging clinical and business stakeholders and end users directly to surface real workflow pain points, not just intake tickets — and translate findings into clear problem statements
● Write product requirements, user stories, and acceptance criteria; partner with technical and vendor delivery teams (Epic/OCHIN, Salesforce, TalkDesk, and others) to scope, sequence, and de-risk delivery
● Maintain the product backlog end to end, balancing new capabilities, enhancements, and technical debt
● Work with business stakeholders to define success metrics and KPIs for each initiative, track adoption and outcomes post-launch, and use that data to inform the next iteration of the roadmap
● Partner with PACE Center Leadership, the Office of the CMO, Population Health, Practice Operations, Finance, Strategy, Growth, and Health Plan Operations to align product direction with business strategy
● Partner with Operations leadership to drive adoption of new rollouts — closing training gaps through documentation, demos, and change management
● Coordinate with multiple vendors, escalating to leadership when vendor needs to be held accountable to scope, documentation, and delivery timelines
● Facilitate stakeholder and steering meetings, and represent the voice of the product to senior leadership
● Operate with a high degree of ownership and autonomy, exercising sound independent judgment to identify issues, evaluate options, make recommendations, and drive matters to resolution
Qualifications
● Aligns with our purpose and our values, and is excited about living those out in daily practice
● 3+ years as a Product Manager or Product Owner, ideally in healthcare, health tech, or another complex, highly regulated operational environment
● Demonstrated ability to run discovery, write clear requirements/user stories, and prioritize a backlog using a defined framework rather than by loudest voice
● Comfort using success metrics and relevant data to make and defend prioritization calls for review/sign-off
● Excellent communication skills and ability to facilitate cross-departmental meetings, including senior leadership as well as line-level staff
● Strong documentation skills, especially product requirement docs, roadmap artifacts, and PowerPoint/board-ready materials
● Proficiency with MS Office suite, backlog/roadmap tooling, and visualization tools
Nice to Have
● Experience with Salesforce, TalkDesk, and/or Epic
● Formal product management training or certification (e.g., CSPO, Pragmatic Institute)
● Experience with Agile methodology, including working as or with a SCRUM team
● Experience with healthcare interoperability standards (e.g., FHIR) or EHR data models
● PACE experience
