About the Opportunity:
The ADMISSION COORDINATOR (HOSPICE) plays a key role in managing hospice referrals and admissions, serving as the primary coordinator who ensures timely and accurate processing of client intake. This position directly impacts client and family experiences by facilitating the initiation of hospice services and fostering collaboration between clinical, sales, and operational teams.
Responsibilities:
Serve as lead coordinator for hospice referral and admissions processing.
Receive and manage referrals from physicians, hospitals, facilities, and community resources.
Collaborate with departments to obtain billing and clinical preadmission information.
Communicate hospice benefits and services to clients and families.
Coordinate care start with referral sources, insurance, and internal teams.
Maintain accurate data entry and documentation in hospice software.
Monitor referral calls, faxes, and portals, responding promptly.
Ensure compliance with applicable policies and regulations.
Support families and clients at hospice admission.
Build effective working relationships across functional teams.
Requirements:
Bachelor’s degree.
Minimum two years hospice experience or related medical office, customer service, or intake role.
HIPAA-compliant home office setup.
Positive attitude and strong communication skills.
Ability to work in high-stress environments.
Proficiency in Microsoft Office Suite.
Familiarity with hospice benefits, Medicare and Medicaid guidelines.
Ability to coordinate with various healthcare stakeholders.
Willingness to learn hospice regulatory standards.
High attention to detail and organizational skills.
Ability to handle confidential information with discretion.
Pay Range and Compensation Package:
